DATA SUBJECT APPLICATION FORM
Pursuant to the Personal Data Protection Law No. 6698 (KVKK), individuals defined as “Data Subjects” are granted certain rights regarding the processing of their personal data under Article 11 of the law, titled “Rights of the Data Subject.” In this context, in accordance with Article 13 of KVKK, the Data Subject must submit their requests to exercise these rights to Sami Trafo Makina İnşaat İmalat Taah. Genel Tic. Ltd. Şti (the “Company”), acting as the Data Controller, either in writing or by other methods determined by the Personal Data Protection Board.
Applications made in writing to our Company should be submitted using this form through one of the following methods:
- By registered mail with return receipt
- Through a notary
- By signing this form with a “secure electronic signature” as defined in the Electronic Signature Law No. 5070 and sending it to our Company’s registered electronic mail (KEP) address.
Application Submission Methods:
You may submit your written applications with a wet-ink signature in person or via notary to Hacı Sabacı OSB Anafartalar Caddesi No:11 Sarıçam/Adana, Turkey or send them to our KEP address (samitrafomakina@hs01.kep.tr) or email address (info@samitrafo.com) after signing with a secure electronic signature.
Our Company will process applications regarding the exercise of rights under Article 13 of the Law as soon as possible and at the latest within thirty (30) days from the date of receipt, free of charge. However, if the process incurs additional costs, our Company may request a fee from the applicant in accordance with the tariff determined by the Personal Data Protection Board. If the Company accepts the request or rejects it by providing reasons, it will notify the Data Subject in writing or electronically.
If the information and documents submitted by the Data Subject are incomplete or unclear, or if it is necessary to verify whether the applicant is the actual owner of the personal data in question, our Company may request additional information/documents from the applicant.
